The NTSB has released its preliminary report on the UPS plane crash in Louisville, Kentucky, in November 2025. Jeff Ostroff presents the key findings from the investigation into the UPS plane crash, detailing the sequence of events and potential causes. The NTSB's initial findings confirm that the investigation into the November 4, 2025, crash of the McDonnell Douglas MD-11F in Louisville, Kentucky, is centered on a significant structural failure. Airport surveillance video captured the moment the left engine and its pylon separated from the wing shortly after the aircraft began to lift off the runway. Investigators have examined the recovered engine pylon and discovered evidence of metal fatigue. Specifically, the report details fatigue cracks on the lugs of the pylon's aft mount, which is the structure that connects the engine pylon to the wing. The NTSB also released still images from the airport surveillance video showing the engine separating from the aircraft.
